I have been talking about this error for years, and have even published a paper on it. I critiqued a single example of it, and then discussed how the problem was rampant in massage therapy research. Based on the Nieuwenhuis paper, apparently it’s rampant elsewhere as well, and that is really unfortunate. The fact is, I do already do all those ideal things occasionally, but sometimes it feels as if being in the world is too much, and I need to disappear from it by losing myself in a screen. Trigger points or stress points may also cause muscle soreness and decreased flexibility. These points are specific spots in muscle and tendons which cause pain when pressed, and which may radiate pain to a larger area. They are not bruises, but are thought by some to be small areas of spasm. Trigger points may be caused by sudden trauma (like falling or being hit), or may develop over time from the stress and strain of heavy physical exertion or from repeated use of a particular muscle.

Massage is not a detox treatment. If anything, it’s the opposite! Post-massage soreness and malaise (PMSM) is probably caused by mild rhabdomyolysis (“rhabdo”). And yet some medical benefits are plausible despite the lack of evidence. For instance, many apparently successful treatments may be due to the effects of pressure on “muscle knots,” which are a likely factor in many common pain problems, but poorly understood (and difficult to treat). And regardless, the effects on mood and mental health are so profound that patients can’t really lose — it’s a valuable service whether it “works” for pain or not.
Another serious general concern about the quality and effectiveness of massage therapy is that there is so much emphasis placed on specific, branded “techniques” and styles. The massage world is fragmented into dozens or even hundreds of these, depending on how you count. It is especially troubling that so many proprietary techniques are hyped as “advanced” and taught in place of genuine continuing (academic) education. This is the serious problem of certification rackets or “modality empires” — selling credibility to therapists in the form of certifications for a treatment method. These techniques are proprietary and profit-motivated, and usually championed and promoted by a single entrepreneur who gets treated like a guru and has legions of dedicated followers (who tolerate criticism rather poorly).

Does massage therapy “work”? What do massage therapists say that they can do for people and their pain, and is there any scientific evidence to support those claims? Massage is a popular treatment for low back pain, neck pain, and tension headaches — can it actually treat them, or does it just pleasantly distract patients and maybe take the edge off? In this article, I examine massage therapy in the light of science — not “objectively,” but fairly.1 I go out of my way to be critical of my former profession — I consider it an ethical duty. Health professionals must be self-critical and critical of each other: that is how we improve.2 And, alas, massage therapists are guilty of an astonishing amount of bullshit.

A licensed massage therapist should ask you about your health history before the massage. Thai massage may not be safe for someone with health conditions such as disk herniation, osteoporosis, recent surgery, or cardiovascular disease. If you're considering trying Thai massage, it's a good idea to check with your healthcare provider before getting treatment to discuss whether it's appropriate for you.
“Massage therapy may be effective for treatment of chronic back pain, with benefits lasting at least 6 months”: A strongly positive summary, barely tempered by the word “may.” Unfortunately, their evidence does not really support such a sunny conclusion. In fact, their data showed that the benefits of massage were minor to begin with, and barely detectable after six months. Worse still, there’s that lack of blinding thing again. They concede the flaw but fail to acknowledge its serious implicates: if anything, as with Michele Preyde’s study, it flips the story, from good news to a depressing evidence of absence.
Massage therapists who have received specialized training and are certified in prenatal massage know how to position and support the woman's body during the massage, modify techniques, and avoid certain areas and techniques during pregnancy. Most will have a special table that allows the woman to rest comfortably and safely during the massage. Ideally, you should seek out a practitioner who is experienced and licensed in prenatal massage.

Leave the past behind, and think of what you can do in the future to put those memories to rest. It may help to do a physical action to lay that memory to rest, or to let your mind let it go. Get a chalkboard and draw the memory, or write, on it, then ritualistically erase it, knowing it wil leave your mind as it leaves the chalkboard. Or, write it on a piece of paper, and throw it into a fire to be consumed, thus eradicating it from your mind.
Whether it is the actual goal of therapy, or just an intriguing side effect, the sensations of massage can change our sense of ourselves, how it feels to be in our own skin, and perhaps bump us out of some other sensory rut82 — and that, in turn, may give us some leverage on our emotional ruts. The sensory experience may have complex effects on emotions and cognition. And personal growth and emotional maturation probably have some clinical relevance to recovery and healing (see Pain Relief from Personal Growth: Treating tough pain problems with the pursuit of emotional intelligence, life balance, and peacefulness).

Ergonomic chairs serve a similar function as a massage table. Chairs may be either stationary or portable models. Massage chairs are easier to transport than massage tables, and recipients do not need to disrobe to receive a chair massage. Due to these two factors, chair massage is often performed in settings such as corporate offices, outdoor festivals, shopping malls, and other public locations.
Connective tissue stimulation. A lot of therapists are keen on stretching connective tissues — tendons, ligaments, and layers of Saran wrap-like tissue called “fascia.” I’m not a huge fan of this style, but certainly it’s a way of generating many potent and novel sensations, which may be inherently valuable to us — another form of touch. Although “improving” the fascia itself is implausible and unproven, perhaps fascial manipulations affect bodies indirectly, just as a sailboat is affected by pulling on its rigging. People have written whole books full of speculation along these lines. So, as long as the sensations are not like skin tearing (that’s an ugly pain for sure), you might choose to tolerate this kind of massage if it seems to be helping you.

I have found that not running away from things, but confronting them and reflecting on them, can feel as exhausting as the running itself. It is difficult, disturbing work. But in a room with someone who can listen and help me to make sense of things, it can also be a relief. Morgan tells me: “We have all these various ways of distracting ourselves from the most important fact of life – that we live, and then we die. Having a mind to help you think about things, having a person who can think deeply about things with you, is a way to manage this very frightening fact of life.”

It has rarely been directly tested and it has never been done well (and never for back pain specifically, which is probably of the greatest interest).78 If you squint optimistically, you could call the best of the evidence “promising.” You could say that where there’s smoke, there’s fire. But it’s like the smoke from last night’s campfire — more of a smokey smell than a smoke where any fire might be hiding. Dial up even a mild cynical impulse, and the evidence collectively looks more like a damning failure to produce any clearly good news.
When you think of a massage, you probably think of soothing music, a gentle brush of hands softly kneading the stress from your shoulders, maybe even of a loved one offering to rub your back after a long day at work. While some massages can be soothing, and rely on gentle touches to work out a client’s stress or anxiety, there are other massages that have a little more grit to them. Working out these harder to reach muscles will require more pressure, making the Deep Tissue massage slightly uncomfortable, gritty and highly effective.
Expect to work hard breaking into the sports environment. Get additional training and develop your expertise in orthopedic massage and functional assessment, clinical massage, myofascial work and stretching. Another interesting indirect example: stretching massages muscles with movement, and so it may feel good for some of the same reasons and share some of the same benefits. In treatment, physical pressure is applied to acupuncture points with the aim of clearing blockages in those meridians. Pressure may be applied by fingers, palm, elbow, toes or with various devices.
There are a lot of relaxation techniques out there that we can all learn and practice. However, I have found that many of my clients (and myself) struggle with the way these are presented, and they also have problems motivating themselves to use them. In response to that, I researched the types of relaxation techniques that have the most research support, and I developed a five-step sequence that anyone can do to feel more relaxed in minutes, minus the new-age vibes.
The approval of skeptics doesn’t mean that massage “works,” and they may actually be giving massage more credit than it deserves. At TAM7, I repeatedly explained to horrified skeptics — who were trying to give me the benefit of the doubt — that my colleagues routinely either sell or endorse virtually every imaginable form of alternative health care, including the silliest: ear candling, crystal therapy, iridology, gong therapy (look it up!) … you name it, there’s a goofy therapy that many massage therapists “believe” in.

Our mind and body respond to scent as the brain reacts directly to smell.  Aromatherapy is based on exactly that. Different scents have different effects, and there are volumes of research about which scent does what. Some have the ability to calm and relax, which is exactly why aromatherapy could be very useful for people with anxiety. Use an oil diffuser to spread the aroma of your choosing around your household or simply add a few drops of oil to your bath or onto your skin. Take care using undiluted oils on your skin, however.  Read the instructions carefully as some can cause irritation.

According to a report by Ofcom this summer: “Most people in the UK are dependent on their digital devices and need a constant connection to the internet.” It found that 78% of us now own a smartphone – rising to 95% of 16- to 24-year-olds. We check these phones on average every 12 minutes of our waking lives, with 54% of us feeling that the devices interrupt our conversations with friends and family, and 43% of us feeling that we spend too much time online. We can’t relax with them, and we don’t know how to relax without them. Seven in 10 of us never turn them off.

Seriously, walk away from the screen(s). In fact, once you finish reading this article, you can walk away from this screen, too. Today, there’s so much time spent taking in information. You read the news, have your most personal conversations, and work, all from the same little screen. So, leave it behind. Taking regular breaks from your phone and computer can help reset your brain and bring relief. And doing so before bed (at least 45 minutes before) will help you drift to slumber without a heavy mind. 

Two general effects [of massage, MT] are well-supported by scientific data and widely agreed-upon by MT researchers. Quantitative research reviews show that a series of MT treatments consistently produces sizable reductions of depression in adult recipients. The effects of MT on anxiety are even better understood. Single sessions of MT significantly reduce state anxiety, the momentary emotional experiences of apprehension, tension, and worry in both adults and in children, and multiple sessions of MT, performed over a period of days or weeks, significantly reduce trait anxiety, the normally stable individual tendency to experience anxiety states, to an impressive degree in adults.
Massage may be an appropriate technique for helping certain sports injuries, especially muscle injuries, to heal. When treating an injury, however, it is best to seek advice from a qualified sports therapist or a specialist in sports medicine before performing any massage. Certain ligament and joint injuries that need immobilization and expert attention may be aggravated by massage.
Challenges include breaking into the sports arena. Often, massage is not viewed as a primary service. If you do get in with a team or individual, the travel schedule can be daunting; long hours, little sleep, and being away from your family and business can be really difficult. Finally, the work can be very physically challenging. This can take a toll on the massage therapist, so exceptional body mechanics and self-care are essential.
Leave the past behind, and think of what you can do in the future to put those memories to rest. It may help to do a physical action to lay that memory to rest, or to let your mind let it go. Get a chalkboard and draw the memory, or write, on it, then ritualistically erase it, knowing it wil leave your mind as it leaves the chalkboard. Or, write it on a piece of paper, and throw it into a fire to be consumed, thus eradicating it from your mind.
A great may of the massage modality empires are based on a basic guiding principle or school of thought I call “structuralism” — an excessive preoccupation with biomechanical and postural factors in pain problems, AKA the biomechanical bogeymen. Structuralist techniques are all fixated to some degree on straightening or improving your meat, because they believe that you are crooked or unbalanced in some way. This notion is easy to sell, but the entire school of thought has little merit. It is debatable at best — and debunked nonsense at worst. We often forget to focus on the simplest, shortest (and one of the most restorative) activities available to us: our breath. Yoga teacher Anna Guest-Jelley suggested taking five deep breaths. “As you do, notice the natural pause between your inhale and exhale, and then between your exhale and your next inhale. Your body has a built-in break — how great is that?!”


Only the statistical significance of the results is touted in the abstract, not their size. This almost always means a real effect that was too small to emphasize. So I read the full paper and, sure enough, the effects of massage were positive but modest at best (and in many cases trivial). There were a lot of measures of success, and none changed all that much. The phrase “damned with faint praise” comes to mind, as it so often does in massage science.

Due to this techniques harsher pressure, there are certain individuals who should seek pain relief elsewhere. Deep Tissue massages can be dangerous for clients who have blood clots, as the pressure and movements might cause the clots to be dislodged. If you have had blood clots in the past, currently have blood clots or are at risk of forming blood clots, you should consult a doctor before pursuing a Deep Tissue massage. If you are currently recovering from recent surgery, chemotherapy, radiation or are suffering from osteoporosis you too should speak with a doctor before experimenting with Deep Tissue massages. As this is particularly high pressure massage, you should not allow rashes, wounds, tumors, hernias, etc. to be massaged directly. Expecting mothers should also avoid Deep Tissue massages; instead, ask your doctor about therapists that specialize in pregnancy massage techniques.
There are a few “medical” massage therapists out there with some training in orthopedics and rehabilitation. My education in massage therapy here in British Columbia, Canada, was three years long — the longest massage therapy training program in the world. There are also a few other places with two-year programs. A massage therapist with this level of education is certainly the kind that patients should seek out if they want massage as a treatment.

Have you ever smiled even when you were feeling bad? That’s highly unlikely. Make yourself to smile more. Make a mental file of people, places, and things that make you smile.  Pull something out of this mental file when you are feeling overwhelmed.  The positive energy will transfer to your surroundings and echo back to you. In this study participants demonstrated that forcing a smile resulted in feeling more relaxed and an overall greater positive attitude.

For instance, we know that chronic pain in particular tends to get increasingly disconnected from reality (central sensitization), and safe, pleasant, interesting sensations may help us get back on track.12 Such a benefit is both highly plausible and highly speculative. Sadly, there is an absence of useful evidence on the topic. We know many chronic pain patients are drawn to massage like bees to flowers, but we don’t know how much relief is actually possible.
Sometimes it really helps to know that you are not alone. There are many people who have similar issues and find comfort and help with each other. Support groups are amazing when it comes to sharing feelings with like-minded people. Find a support group nearby and attend their meetings or if that may seem a bit too much at this point, find and join a private chat group on social media.

Plus, this is not the kind of study where a large number of subjects is needed to be convincing. Of course, replication and more subjects are always a critical part of science. But the claim of detoxification is what we call a “brittle” claim — it breaks easily, because anything less than a clear positive effect is not enough to impress anyone. For brittle claims, even just a lack of effect is always news, because there should be a worthwhile effect, according to the claim. In this case the claim is that massage meaningfully reduces lactic acid … and in this experiment, it didn’t just fail to have an effect, it had the opposite effect. Panic attacks have certain patterns. Time how long it takes to build up, the duration of the panic attack, how long it takes to calm down and at what point you are finally able to resume your normal activity. It may not be easy doing this, but timing your panic attacks will enable you to have better control over it the next time it happens. You will also be able to tell those around you before an attack occurs, how long it is liable to last, and how they can help you.
