I’ve often said that massage therapy research is in its early stages. And after thinking about that more today, I’ve realized it’s worse than that. Massage therapy research is stunted, and not showing signs that it is ready to progress. Some might disagree, and would point to the increasing number of massage therapy studies. (I’ve charted it myself in at least one paper I’ve published, and there is no doubt that the number of papers on the subject is increasing.) But I would counter by noting that there is no discussion in the field. The studies are conducted and published in isolation. They are not often being critiqued, and researchers with different theories and perspectives are not addressing each other in the literature or even at conferences. 

Some research has shown that massage therapy can be effective during pregnancy due to its anxiety-lowering effects, and women who receive massages before and during labor tend to experience decreased depression, anxiety, and leg and back pain. Results from one study found that women who received massages prior to and during labor had significantly less pain, plus their labors were on average three hours shorter and required less need for medication.

What do the best studies say? One of the best, a Canadian experiment conducted by Michele Preyde way back in 2000,40 was a test of “comprehensive massage therapy” as delivered by well-trained Ontario therapists,41 in six sessions in a month for 25 cases of sub-acute low back pain (non-chronic, but not brand new cases either). This treatment regimen was compared to massage alone, remedial exercise and posture education alone, or some useless laser therapy. Massage alone had “considerable benefit,” just enough to be considered clinically significant; adding exercise prescriptions (and posture education, but that probably wasn’t a difference maker) improved on those results even more, pushing them comfortably into clinical significance.


Research shows that listening to soothing music can lower blood pressure, heart rate, and anxiety. “Create a playlist of songs or nature sounds (the ocean, a bubbling brook, birds chirping), and allow your mind to focus on the different melodies, instruments, or singers in the piece,” Benninger says. You also can blow off steam by rocking out to more upbeat tunes -- or singing at the top of your lungs!
Adequate recovery is also a major factor in avoiding the over-training syndrome. Over-training is characterized by irritability, apathy, altered appetite, increased frequency of injury, increased resting heart rate, and/or insomnia. It occurs when the body is not allowed to recover adequately between bouts of heavy exercise. Of course, back in the day when people were mostly hunters and gatherers, these physiological changes would help our ancestors run from predators or environmental hazards. And maybe back then, people didn’t know how to relax. Maybe they weren’t really supposed to. But today, people aren’t asked to regularly respond to such great threats, and knowing how to relax may be a better modern survival tactic than a ready stress response.
And yet some medical benefits are plausible despite the lack of evidence. For instance, many apparently successful treatments may be due to the effects of pressure on “muscle knots,” which are a likely factor in many common pain problems, but poorly understood (and difficult to treat). And regardless, the effects on mood and mental health are so profound that patients can’t really lose — it’s a valuable service whether it “works” for pain or not.
Thai massage has been practiced in America since the 1990s when Westerners began practicing it. Thai massage is more commonly found than it used to be at American spas, but you can't find it everywhere for two reasons. First, the spa needs a room with a large, padded mat in order to offer Thai massage. It's easiest if the room is always set up for Thai massage, and yet it is more of a specialty request. It just makes more economic sense to have the room set up with a table for a Swedish massage.

This geeky basic neurology experiment produced a rough estimate of the density of nerve endings in human glabrous (hairless) skin: about 6000 per square centimetre, so a whole hand probably contains about as many as the maximum capacity of the largest stadiums in the world. They measured an average nerve diametre of about 3 thousandths of a millimetre.
In South Africa, massage is regulated, but enforcement is poor. The minimum legal requirement to be able to practice as a professional massage therapist is a 2-year diploma in Therapeutic Massage and registration with The Allied Health Professions Council of SA (AHPCSA). The 2 year qualification includes 240 credits, about 80 case studies, and about 100 hours community service.

An Indian physician named Shivago Kumar Bhucca, a contemporary of the Buddha, is often credited with developing Thai massage. The impetus was to give monks and nuns the flexibility to sit for long hours in meditation. Whatever its genesis, massage was considered such a crucial aspect of medical treatment in Thailand that until the early 20th century, the Thai Department of Health included an official massage division. Thai massage is based on releasing blockages along ten lines of energy called sen, which are similar to the meridians of traditional Chinese medicine. The technique incorporates stroking and kneading of muscles, manipulation of joints, and pressure applied to specific points in order to balance the body’s four elements—earth, water, fire, and air. But there’s also rocking, breathing, and lots of stretching—Thai massage is often referred to as “lazy yoga.”


Meditation is often seen as a great way to relax, and many studies back up that idea. In a meta-analysis looking at several studies of relaxation training in people with anxiety, the observed effects of meditation were greater than those observed for other intentional relaxation techniques, such as autogenic training (a process that involves making your body feel heavy and warm) and applied relaxation. Other studies have also outlined its effect as an anti-anxiety and antidepressant method.

Meditation in its purest form involves focusing on one thing to clear your mind. Deep tissue technique uses very little to no lubricant so that the muscles can be hooked or grabbed, thereby stretching and lengthening them, and to separate adhered muscle compartments. Strokes will be considerably slower and possibly shorter as the therapist waits for a slow release of tension. Some areas may be skipped so more time can be spent on specific areas of need. Doing this provides better alignment of the muscles and less restriction in the joints, thereby improving their movement and function. It is a massage in which the primary goal is less about general relaxation and more about promoting change in the actual structure of the body.

Shoulder pain. A 2017 study published in the Journal of Physical Therapy Science reports that shoulder pain is one of the most common musculoskeletal disorders, affecting as many as 66.7 percent of the population. The study goes on to say that massage therapy can often help reduce this type of pain, sometimes in a matter of days, though 36 sessions appeared to offer the greatest level of relief.
One narrative review in Open Access Journal of Sports Medicine explains that the impact of using these two modalities combined are somewhat inconclusive, mainly due to research limitations; however, after looking at 21 randomized controlled trials, the author ultimately concluded that “the effects of cold and static compression are clearly better than no treatment.”
If you feel that you would benefit from experiencing a Deep Tissue massage, do some research into local spas around you that offer the treatment. Make sure that your therapist is certified and has been trained in this technique. As always, check with your doctor before giving this massage a try. Be sure to share your concerns with your therapist, go into detail as to what has brought you in to seek the treatment and do not be afraid to speak up if the pressure becomes too uncomfortable. Remember, more pain does not mean the massage is working.
The Touch Research Institute has conducted about many dozens of small studies showing the positive effects of massage and touch therapies on many conditions. It’s all a little too good to be true (or all true). I think the Touch Research Institute designs studies in such a way that a positive spin on touch therapy is inevitable. For instance, many of their papers are particularly afflicted by two statistical errors: confusing statistical and clinical significance (or just ignoring the difference when convenient), and comparing the wrong things to arrive at so-called “significance” (see Statistical Significance Abuse: A lot of research makes scientific evidence seem more “significant” than it is.)
