The physiological effects are a bit of a moot point: if the pressure doesn’t suit you, you’re not likely to continue with the therapy. The exception is the patient who is willing to put up with intense pain long enough to find out if there appears to be a therapeutic effect afterwards, which there may be. It’s important to be open with your massage therapist about the level of pressure and discomfort you wish to endure. This may be different for certain areas and throughout the massage, feel free to communicate with your massage therapist before and during the massage. Some massage therapists find pain to be counterproductive to the process and expect you to speak up if the pain is too much.

The reason the Pressure Question exists is that it’s hard for patients to tell the difference between nasty pain that might be a necessary part of therapy, and ugly pain that is just abusive. Not everything that hurts is therapeutic, but not every therapeutic procedure is painless! How can we tell if an intense massage technique is therapeutic or not?

Most deep tissue massages normally focus on major muscle groups — such as the neck or lower back — along with joints and tendons that are susceptible to straining or injuries. Certain areas of the body that tend to tense up in times of stress, including the shoulders, neck and hips, can often benefit the most from this type of deep manipulation. Many people consider “sports massages” to be a form of deep tissue massage, which involves physical treatment primarily to neuromusculoskeletal systems to treat pain and disability, improve muscle recovery and joint mobilization, and prevent injuries.
Wat Pho, the center of Thai medicine and massage for centuries, opened the Wat Pho Thai Traditional Medical and Massage School in 1955 on the temple grounds, the first such school approved by the Thai Ministry of Education. Training and certification standards for massage therapists are all over the map, and most practitioners have barely scratched the surface of musculoskeletal medicine. The profession is rife with pseudoscience like crank theories about the causes of pain, the myth that massage detoxifies, or that painfully “deep tissue” massage is needed to “release” tissues (which is stressful or even harmful for many patients).

Everyone I know, including myself, is busy and distracted on an infinite loop. It’s a curious conundrum, considering we’re all armed with more than enough information on how to de-stress. But the “surprising benefits” of exercise, meditation and work-life balance, documented ad nauseam, are beginning to feel like drawn-out humblebrags. Sometimes, they just seem insurmountable. I’ve often said that massage therapy research is in its early stages. And after thinking about that more today, I’ve realized it’s worse than that. Massage therapy research is stunted, and not showing signs that it is ready to progress. Some might disagree, and would point to the increasing number of massage therapy studies. (I’ve charted it myself in at least one paper I’ve published, and there is no doubt that the number of papers on the subject is increasing.) But I would counter by noting that there is no discussion in the field. The studies are conducted and published in isolation. They are not often being critiqued, and researchers with different theories and perspectives are not addressing each other in the literature or even at conferences.
The University of Maryland Medical Center reports that deep-tissue massage is more effective and affordable for relieving chronic pain than conventional medical remedies. Because deep-tissue massage increases the flow of blood through through the body, it helps reduce the inflammation that causes pain. Deep-tissue massage can also help alleviate muscle tension that is often a side effect of chronic pain by loosening the tight tissue clusters.
Bad pain. Bad pain comes with no obvious, immediate benefits. If there is anything good about it, there is no way to tell from the sensation at the time. Bad pains are usually sharp, burning, or hot. Such pain is usually caused by excessive but harmless pressure. As bad as it feels, it probably won’t hurt you — maybe a little bruising — but there’s also a good chance that it won’t be therapeutic either. The big question about bad pain is whether or not it is ever justified.

In India, massage therapy is licensed by The Department of Ayurveda, Yoga & Naturopathy, Unani, Siddha and Homoeopathy (AYUSH) under the Ministry of Health and Family Welfare (India) in March 1995. Massage therapy is based on Ayurveda, the ancient medicinal system that evolved around 600 BC. In ayurveda, massage is part of a set of holistic medicinal practices, contrary to the independent massage system popular in some other systems. In Siddha, Tamil traditional medicine from south India, massage is termed as "Thokkanam" and is classified in to nine types, each for specific variety of disease.


Speaking to Grunfeld and Andrew, and hearing their advice (see ) on how to identify different occupations that might relax and reinvigorate me, I begin to feel optimistic. I think back to how I liked to pass the time when I was young; the quiet times sitting reading a book, the rowdier times baking with friends. I resolve to make more time to do the adult versions of these things over the next year – then realise I am making excuses. If I could redirect the evenings I am already wasting on screens, that would be a good start.
Early mobilization and range of motion exercises, for instance, will be taught by competent massage therapists to clients with cervical injuries, because they help people get better faster.2627 In fact, the evidence strongly suggests recovery from nearly any injury or surgery is greatly facilitated by early mobilization.
