I think it is more that they are circumspect than pessimistic. Speaking as a scientist, we are very careful to guard against declaring a finding if there is even a small risk of it being a false positive. Unlike these clever models, though, we can’t turn it around to see what’s really going on. And trying to see through the illusion, trying to believe that there’s nothing much actually wrong with our tissues (often true), is even more difficult than seeing through these illusions. But that challenge is what recovery is all about: trying to change our expectations and point of view with interesting new sensations and movements. Some procedural notes: swelling was measured all fancy-like, “using bioimpedance spectroscopy and volume.” All patients “followed the standard rehabilitation program of the department,” including ice, passive motion, strengthening, and getting walking. Massage is not a detox treatment. If anything, it’s the opposite! Post-massage soreness and malaise (PMSM) is probably caused by mild rhabdomyolysis (“rhabdo”). True rhabdo is a medical emergency caused by muscle crush injuries. But milder stresses cause milder rhabdo — even just intense exercise can do it. And massage! We know this from a good formal case study, several informal ones, common exertional rhabdo, and the similarities between PMSM and ordinary exercise soreness. For instance, we know that chronic pain in particular tends to get increasingly disconnected from reality (central sensitization), and safe, pleasant, interesting sensations may help us get back on track.12 Such a benefit is both highly plausible and highly speculative. Sadly, there is an absence of useful evidence on the topic. We know many chronic pain patients are drawn to massage like bees to flowers, but we don’t know how much relief is actually possible.
The following relaxation process is all about taking steps to change your physiology when you are tense, anxious, or angry. This is basically done by shifting attention and taking control of breathing as a way to jump-start the parasympathetic nervous system. There is no magic, just basic steps (some that you already do) that should result in you feeling more relaxed, quickly. I encourage you to practice doing the full sequence at least once a day for two weeks during times of lower tension before you start using it in more urgent situations. Additionally, there is no set amount of time that this takes. It could last as little as a minute or two, or go on as long as you like.

I saw a few chiropractors and acupuncturists. But despite some initial short relief, their work seemed to lose effectiveness after a few visits. One of the greatest Persian medics was Avicenna, also known as Ibn Sina, who lived from 980AD to 1037AD. His works included a comprehensive collection and systematisation of the fragmentary and unorganised Greco-Roman medical literature that had been translated Arabic by that time, augmented by notes from his own experiences. One of his books, Al-Qānūn fī aṭ-Ṭibb (The Canon of Medicine) has been called the most famous single book in the history of medicine in both East and West. Avicenna excelled in the logical assessment of conditions and comparison of symptoms and took special note of analgesics and their proper use as well as other methods of relieving pain, including massage.
Lymphatic drainage is an interesting example of a specific massage technique, allegedly good for one thing and not much else: its purpose is to reduce swelling. By reputation, it’s the best treatment option for patients suffering from lymphoedema, a serious complication of mastectomy and other surgical procedures. But it’s also obscure, technical, and practiced by no more than a few hundred therapists globally. It’s not “massage therapy” per se; just a specialized tool that a tiny group of professionals specialize in, some of whom happen to be massage therapists. Oh, and bad news: there’s also recent evidence that it does not work,28 or not nearly as well as we’d like.29

Traditional Thai massage uses no oils or lotions. The recipient remains clothed during a treatment. There is constant body contact between the giver and receiver, but rather than rubbing on muscles, the body is compressed, pulled, stretched and rocked. Thai stretches are often very similar to yoga poses. Back walking may be performed during a Thai massage as well.
Most other evidence about massage benefits is indirect and/or so weak it’s meaningless. For instance, there are a few scraps of evidence — not nearly enough, but better than nothing — that rubbing and stretching soft tissue can reduce joint pain and stiffness.8 (Note that stiffness is a symptom, a sensation of discomfort with movement that may or may not involve any actually limited movement. 2. Grounding: the second step helps shift your attention to how you are connected to your environment. Since relaxation is a physiological process, it is important to direct your attention to your senses. So for this step, intentionally notice ways you are connected to your surroundings. For me right now, that would mean I intentionally notice my feet on the floor, my back against the chair, and how my sweater feels on my arms.
Massage is a profoundly valuable service regardless of what specific effects it does or does not have on pain, tissues, or pathologies. A pleasant, relaxing experience may have any number of minor therapeutic benefits, such as bringing your blood pressure down. However, the subtler benefits of massage extend well beyond that, into the territory of emotional and psychological benefits that are virtually impossible to define or measure — and surprisingly potent.

This is not only an inaccurate and potentially harmful picture of this type of therapy, but such misguided practices can bruise muscles, elicit a defensive reaction in a client’s body, and worsen pain cycles. Properly executed deep tissue work should not cause the client to grit their teeth in agony as the therapist coerces the body into submission! If you find yourself clenching, shortening or holding your breath, or gritting your teeth, then it’s TOO DEEP. Even when it gets intense, it should not go above about a 7 on the pain scale: enough to “hurt so good,” but not enough that you want to leap off the table (and never come back).
