I agree with almost every detail of the article and wrote a letter of support to Dr. Barrett, which is published as an addendum to it. That said, the article does neglect some nice things that can be said about massage therapy, and it contains a few minor errors. But I applaud the intent and embrace and welcome most of the criticism. I wish it weren’t mostly true, but I believe that it is.


Massage therapists will use their fingers, thumbs or occasionally even elbows to apply the needed pressure. It is especially helpful for chronically tense and contracted areas such as stiff necks, low back tightness, and sore shoulders. Some of the same strokes are used as classic massage but the movement is slower and the pressure is deeper and concentrated on areas of tension and pain.
Reflexology is best for people who are looking to relax or restore their natural energy levels. It’s also a good option if you aren’t comfortable being touched on your entire body. Reflexology uses gentle to firm pressure on different pressure points of the feet, hands, and ears. You can wear loose, comfortable clothing that allows access to your legs.
Our mind and body respond to scent as the brain reacts directly to smell.  Aromatherapy is based on exactly that. Different scents have different effects, and there are volumes of research about which scent does what. Some have the ability to calm and relax, which is exactly why aromatherapy could be very useful for people with anxiety. Use an oil diffuser to spread the aroma of your choosing around your household or simply add a few drops of oil to your bath or onto your skin. Take care using undiluted oils on your skin, however.  Read the instructions carefully as some can cause irritation.

If this theory is correct, or even half-right, it would go a long way to explain the strong appeal of massage — maybe it actually can take the edge off a great variety of problems — but also its inability to work miracles. If trigger points are the main reason massage seems at least a little bit helpful in so many cases, they are also the reason that the results are so unpredictable. The best ways to treat trigger points are simply unknown, and it may be next to impossible. All trigger point therapy is guesswork. Therapists have greatly variable education, skill, and luck in this process. Most simply aren’t that good at it — they can’t be, because muscle pain physiology is obscure, complex, and basically way out of their league.80


Lactic acid is not a dead-end, “bad” metabolic waste product, and it does not cause post-exercise soreness. This is a pernicious and seemingly un-killable myth. It originated with “one of the classic mistakes in the history of science,” according to George Brooks, a Berkley physiologist. I will not give the myth any further air time here. Pressure to meet a deadline can be stressful, but acupressure can help release all that tension. Challenges include breaking into the sports arena. Often, massage is not viewed as a primary service. If you do get in with a team or individual, the travel schedule can be daunting; long hours, little sleep, and being away from your family and business can be really difficult. Finally, the work can be very physically challenging. This can take a toll on the massage therapist, so exceptional body mechanics and self-care are essential.

4. Coaching: once you have the breathing pace down, keep doing it while you move to this step. The key here is giving yourself positive, reassuring, and calm messages, rather than continuing with the tense, anxious, and angry thoughts. When I do this, I think things like "I can get through this. It will be OK. I can handle whatever happens. I am going to calmly do my best." Everyone will have a different way of doing this, and some people like to imagine this in the voice of someone they care about, or with the image of that person telling them those things. Keep doing this along with the breathing until you feel sufficiently ready to reconnect with what you were doing.

Note that this study compares a more vigorous sports massage style with more common Swedish petrissage techniques. Vigorous massage did indeed show significantly increased circulation! However, this technique is rarely used — the vast majority of Registered Massage Therapists in British Columbia rarely treat their clients with vigorous sports massage techniques, yet they still have a habit of claiming that massage increases circulation.

But logistics, economics, and devilish details get in the way. Not many scientists are interested in studying massage, while massage therapists don’t have scientific training. It’s an expensive and overwhelming challenge for a massage therapist to make room in their career for some research — few do it, and hardly any have ever done it well. Even when they do, you’d be amazed how hard it is to even find 100 people with the same problem, so studies of that size are almost never done: instead you get studies of 20 or 30 patients, which isn’t generally enough to prove much. Another challenge is that “massage” can mean so many things that it’s hard to know what is even really being studied (lack of standardization of treatment).4
Having a massage every few weeks is a great way to reduce muscle tension and get pampered at the same time. Massage helps relax the mind and body, and a skilled massage therapist can find your problem spots and get them under control. On average, professional massage therapists charge about a buck a minute — vastly more than millions of people can afford on a regular basis. This economic perspective is often completely ignored in discussions of whether or not massage works. It probably does … but does it work well enough for the price? If massage is to be considered a more cost-effective treatment for any medical problem than napping, we really must establish that it does more — quite a lot more — than just mellow people out.

So the imperfect evidence shows that massage can maybe help low back pain, and yet the world has certainly not been saved from back pain. What’s wrong? Why isn’t massage immediately, completely, and permanently fixing lots of back pain clients? Because there are many kinds of both massage and back pain. Results of therapy vary widely with the skills of therapists, and with the specific kinds of back pain brought to them. And so, on average:

Dr. Stephen Barrett is a prominent anti-quackery activist. Bad pain. Bad pain comes with no obvious, immediate benefits. If there is anything good about it, there is no way to tell from the sensation at the time. Bad pains are usually sharp, burning, or hot. Such pain is usually caused by excessive but harmless pressure. As bad as it feels, it probably won’t hurt you — maybe a little bruising — but there’s also a good chance that it won’t be therapeutic either. The big question about bad pain is whether or not it is ever justified.
• Try to remember what you most enjoyed doing as a child, then identify the most important aspect of that activity and find the adult version. Grunfeld says: “It might be that you can’t remember, and you have to ask friends or family, or look at old photo albums. There are normally themes in all of our lives, and if we’re missing those themes as an adult, it’s almost as if we’re not a whole person.” If you loved playing in the sandpit, you might want to try pottery, or if you liked building things, you might want to make bread.

Injury: in the case of an injury, the recovery treatment will adapt to the healing process of the injury. At the beginning of an injury, massages are more frequent, short and focused on the area. For example, a sprained ankle may need light but bi-weekly work after the acute phase is over. As the injury recovers, massages are more intense, and less frequent. The ankle will receive deeper massages and deeper stretches as it heals. Once the injury is recovered, only one or two check-up massage sessions will be required.

I ended up amassing quite a collection of my own, using them to do chakra balances on people, performing psychic surgery with them, and any number of woo procedures. I also purchased magnetic pads for my massage table. I attended homeopathy workshops. I got heavily into essential oils, which I still love and use today—with the caveat that while I think many of them are useful as folk remedies for various simple ailments, I’m not going to advise someone with cancer that they can cure it with an oil, which unbelievably, I notice massage therapists doing all the time—and worse—on social media.
There are a lot of relaxation techniques out there that we can all learn and practice. However, I have found that many of my clients (and myself) struggle with the way these are presented, and they also have problems motivating themselves to use them. In response to that, I researched the types of relaxation techniques that have the most research support, and I developed a five-step sequence that anyone can do to feel more relaxed in minutes, minus the new-age vibes.

This work has long been regarded as of cardinal importance in the history of physiotherapy because it almost certainly influenced the Swedish founder of the modern phase of the art, Per Hendrik Ling. Cibot had studied at least one Chinese book, but also got much from a Christian neophyte who had become expert in the subject before his conversion."[14]
Some of her patients, Andrew explains, simply never get around to thinking about how they want to spend their time. “People say they are so busy doing the ‘shoulds’,” she says – whether that is working, caring for family or being a part of demanding friendships – that by the time an evening or weekend comes around when they might do what they want, there is no energy or motivation left for anything but “flopping out”. She adds: “That’s a difficulty – because how is life enjoyable or satisfying in the long term if you’re only doing what you should do the whole time?”

When travelling with USA Swimming, a typical day for Olympic Trials, which is a 10-day trip, usually looks like the following: Arrive at pool by 9 a.m. to start 20-minute massage sessions for athletes. According to the National Center for Complementary and Integrative Health (NCCIH), this includes individuals with bleeding disorders, low blood platelet counts, or those who are taking blood thinning medications. When these types of conditions are present, the NCCIH indicates that a sports massage with deep tissue work is generally not recommended.

Cathy Wong explains in her article “Deep Tissue Massage: Everything You Need to Know” on verywell.com that while Deep Tissue massages can be slightly uncomfortable, they have been known to reduce stress hormones and heart rate while releasing oxytocin and serotonin, which allow the client to experience a boost in mood and relaxation. Deep Tissue massages are often used to relieve chronic aches and pain, stiff necks, upper back and lower back pain as well as muscle tightness. Therapists treat such issues by utilizing Deep Tissue massages to break up scar tissue and muscle knots and working out adhesions that might be hindering circulation and limiting movement. 

Massage in China is an extremely popular therapy, the city of Shanghai alone playing host to over 1500 foot massage centers while there are more than 3000 in Shenzhen. It is one of the biggest service industries in China with workers in Shanghai numbering in the tens of thousands.[17] The average rate of pay for a worker in the massage industry in China is over 10000 yuan per month, making it among the highest paid jobs in China’s service sector.[18] China’s massage parlors are frequently linked to the sex industry and the government has taken a number of measures in recent times to curb prostitution and the spread of disease. In a nationwide crackdown known as the yellow sweep ("Yellow" in Mandarin Chinese refers to sexual activities or pornographic content), limitations on the design and operation of massage parlors have been placed, going so far as requiring identification from customers who visit massage establishments late at night and logging their visits with the local police.[19][20]

The therapist might use Swedish massage to stimulate circulation of blood and lymph fluids, and trigger point therapy to break down adhesions (knots in the muscles), and stretching to increase the range of motion. Other techniques could include myofascial release, craniosacral therapy, lymphatic drainage and orthopedic assessment. The therapist should also have a good foundation in hydrotherapy modalities including cryotherapy and thermotherapy, which can help with recovery, repair and healing processes.
According to a report by Ofcom this summer: “Most people in the UK are dependent on their digital devices and need a constant connection to the internet.” It found that 78% of us now own a smartphone – rising to 95% of 16- to 24-year-olds. We check these phones on average every 12 minutes of our waking lives, with 54% of us feeling that the devices interrupt our conversations with friends and family, and 43% of us feeling that we spend too much time online. We can’t relax with them, and we don’t know how to relax without them. Seven in 10 of us never turn them off.

Accept that sleep is a very important part of life. During sleep, your mind continues learning in ways that are not possible during waking hours. Sleep restores and refreshes your body in myriad ways that cannot happen when you're awake. Do not be tempted to devalue the worth of sleep. Moreover, the alleged ability of some people to thrive on four hours sleep per night is the exception, not the rule — most of us need the six to eight hour sleep cycle for full restoration. In addition to the many not-really-massage therapies that massage therapists may offer, there are also many claims that massage therapists make about massage itself that are all-too-questionable. The scientific case against massage largely consists of debunking the list of lame claims that define it to a surprising degree — and if you take them away, there’s not a great deal left. Most are just carelessly perpetuated minor myths. Some are not entirely or definitely wrong, but when presented to patients, are often misleading exaggerations and oversimplifications. For instance, massage probably does sometimes modestly increase circulation — just too little and too erratically to matter. It all adds up to a pattern of intellectual laziness in the profession that undermines its credibility and legitimacy.

Many people just try to work through their stress by ignoring it and hoping the stressors pass quickly, even while stressors build up. Because of this, it's not uncommon to be caught off-guard by stress when you reach the point of feeling overwhelmed or to be stressed to the point that it's taking a toll without realizing that you need to work on managing some of the stress in your life and your psyche.
Perhaps one of the reasons massage reduces depression and anxiety: it’s relaxing. While not proven as well as you might think, it is a pretty safe bet,15 and the idea is further supported by evidence that massage therapy may reduces blood pressure1617 and helps people to sleep, even when they are under the unusual stresses of hospital care.18 These are all unsurprising … and unremarkable. Relaxation is an important component of wellness and pain management, and I do not underestimate its value, but it is hardly curative.
Massage has been shown to reduce neuromuscular excitability by measuring changes in the Hoffman's reflex (H-reflex) amplitude.[90] A decrease in peak-to-peak H-reflex amplitude suggests a decrease in motoneuron excitability.[91] Others explain, "H-reflex is considered to be the electrical analogue of the stretch reflex...and the reduction" is due to a decrease in spinal reflex excitability.[92] Field (2007) confirms that the inhibitory effects are due to deep tissue receptors and not superficial cutaneous receptors, as there was no decrease in H-reflex when looking at light fingertip pressure massage.[93] It has been noted that "the receptors activated during massage are specific to the muscle being massaged", as other muscles did not produce a decrease in H-reflex amplitude.[91]
This review of 35 tests of treatments for delayed onset muscle soreness (DOMS) is strongly consistent with my own past interpretations of the research: basically, nothing works. Massage is “slightly effective” but “its mean effect was too small to be of clinical relevance.” The evidence for cryotherapy, stretching, and low-intensity exercise is not promising, but technically still inconclusive: more study needed.
One narrative review in Open Access Journal of Sports Medicine explains that the impact of using these two modalities combined are somewhat inconclusive, mainly due to research limitations; however, after looking at 21 randomized controlled trials, the author ultimately concluded that “the effects of cold and static compression are clearly better than no treatment.”

In sports massage, the massage therapist generally concentrates on a specific problem area that you present, usually associated with some sort of sports activity, such as running, tennis, or golf. The most important thing with sports massage is that you find a specially-trained massage therapist who has mastery of a wide range of techniques and knows when to use them. In the past, many spas used to put sports massage on their menu as a way to appeal to men. As spas have become more sophisticated, however, they realize they shouldn't list sports massage unless they have some therapists with specialized training.
But whether or not it massage is good medicine is still an open question, and this infographic is really just a bit of mild-mannered propaganda. Amatereurish boosterism never does a profession any favours. Paying lip service to science for promotional purposes cheapens it and impedes progress and understanding. Enthusiastically approving of such poor-quality information is a disturbing sign of how far the profession of massage therapy still has to go before it can be taken seriously as a full partner in health care.

AD 1779: Frenchman Pierre-Martial Cibot publishes ‘Notice du Cong-fou des Bonzes Tao-see' also known as "The Cong-Fou of the Tao-Tse", a French language summary of medical techniques used by Taoist priests. According to Joseph Needhan, Cibot's work "was intended to present the physicists and physicians of Europe with a sketch of a system of medical gymnastics which they might like to adopt—or if they found it at fault they might be stimulated to invent something better. This work has long been regarded as of cardinal importance in the history of physiotherapy because it almost certainly influenced the Swedish founder of the modern phase of the art, Per Hendrik Ling. Cibot had studied at least one Chinese book, but also got much from a Christian neophyte who had become expert in the subject before his conversion."[14]


Sometimes confused with pressure point massage,[10] this involves deactivating trigger points that may cause local pain or refer pain and other sensations, such as headaches, in other parts of the body. Manual pressure, vibration, injection, or other treatment is applied to these points to relieve myofascial pain. Trigger points were first discovered and mapped by Janet G. Travell (President Kennedy's physician) and David Simons. Trigger points have been photomicrographed and measured electrically[71] and in 2007 a paper was presented showing images of Trigger Points using MRI.[72] These points relate to dysfunction in the myoneural junction, also called neuromuscular junction (NMJ), in muscle, and therefore this technique is different from reflexology, acupressure and pressure point massage.


In a typical massage therapy session, you undress or wear loose-fitting clothing. Undress only to the point that you're comfortable. You generally lie on a table and cover yourself with a sheet. You can also have a massage while sitting in a chair, fully clothed. Your massage therapist should perform an evaluation through touch to locate painful or tense areas and to determine how much pressure to apply.

Peer-reviewed medical research has shown that the benefits of massage include pain relief, reduced trait anxiety and depression, and temporarily reduced blood pressure, heart rate, and state of anxiety.[80] Additional testing has shown an immediate increase and expedited recovery periods for muscle performance.[81] Theories behind what massage might do include enhanced skeletal muscle regrowth and remodeling,[82] blocking nociception (gate control theory),[83] activating the parasympathetic nervous system, which may stimulate the release of endorphins and serotonin, preventing fibrosis[84] or scar tissue, increasing the flow of lymph, and improving sleep.[10][85]
Yes, certainly it is a bit exaggerated. It could be a lot worse — hey, at least it’s got references! But it could be a lot better. Citing single cherry-picked studies to support broad treatment claims is weak sauce, even if the picks are good (and clearly not all of these are). The evidence and claims here that are stronger are also less important … and those that are more clinically important are also less sound.
Although there is a lot of negativity — also known as realism — in this article, I also still recommend massage. It has some plausible medical benefits, even if they are inconsistent and unproven. For instance, patients may get at least temporary pain reduction from any kind of stimulation of “muscle knots,” which are a likely factor in many common pain problems — but poorly understood, controversial, and certainly treatment is unreliable in any case. (I summarize this topic below, but it’s such a huge topic that I have several other large pages devoted to trigger points.)
Due to this techniques harsher pressure, there are certain individuals who should seek pain relief elsewhere. Deep Tissue massages can be dangerous for clients who have blood clots, as the pressure and movements might cause the clots to be dislodged. If you have had blood clots in the past, currently have blood clots or are at risk of forming blood clots, you should consult a doctor before pursuing a Deep Tissue massage. If you are currently recovering from recent surgery, chemotherapy, radiation or are suffering from osteoporosis you too should speak with a doctor before experimenting with Deep Tissue massages. As this is particularly high pressure massage, you should not allow rashes, wounds, tumors, hernias, etc. to be massaged directly. Expecting mothers should also avoid Deep Tissue massages; instead, ask your doctor about therapists that specialize in pregnancy massage techniques.
Lymphatic drainage is an interesting example of a specific massage technique, allegedly good for one thing and not much else: its purpose is to reduce swelling. By reputation, it’s the best treatment option for patients suffering from lymphoedema, a serious complication of mastectomy and other surgical procedures. But it’s also obscure, technical, and practiced by no more than a few hundred therapists globally. It’s not “massage therapy” per se; just a specialized tool that a tiny group of professionals specialize in, some of whom happen to be massage therapists. Oh, and bad news: there’s also recent evidence that it does not work,28 or not nearly as well as we’d like.29

The results are underwhelming. Although they did a little better than just guessing, the results suggest that it’s difficult even for expert examiners to detect the location of neck and back pain by feel. As well, they were only attempting to detect the side of pain. Imagine how much worse their performance would have been if they had had to identify the location more precisely, or if the pain could have been anywhere or nowhere. So they barely passed the easiest possible test, and probably would have failed a harder one and done no better than guessing.
While a typical runner’s sports massage focuses primarily on the legs, Denunzio insists on incorporating upper body work as well. As she explains it, “nobody has perfect form, especially when they’re fatigued” and runners can unknowingly tense their upper bodies when working out, which in turn creates tightness in their arms, shoulders and back. Ideally, those areas should receive a little TLC as well.

I recall some heated debates between skeptical students and more “open-minded” instructors and school officials. Unfortunately, such massage therapists are quite rare. Most are poorly trained and uncertified. Most work in spas or resorts and on cruise ships, doing treatments that are infamously fluffy and skin deep, with little therapeutic value other than the comfort of a quiet hour of touching (even though many patients find skin-deep massage to be more annoying than anything else). Most of these therapists are earnest and view themselves as medical semi-professionals, despite their comparative lack of training. It’s actually inappropriate to call them “therapists” at all, and in some places (here) it’s actually illegal — they have to use terms like “bodyworker” or “masseuse.”
5. Emerging: the key in this final step is calmly reentering the world. Rather than just stopping this process and jumping back in, focus on going back to what you need to do with the same peace you might have when you wake up from a nice sleep. Just gently getting back into the flow of your day. This should keep your mind and body both staying in a more relaxed and positive state.
The need for some simple source of relaxation can be seen in the initial surge in popularity of the adult colouring book, as well as last year’s 13.3% increase in sales of books providing spiritual guidance on how to live in a hectic world, and the mindfulness “mega trend” seen in Headspace, the meditation app that has been downloaded more than 15m times. Those of us who spent our money on these products were presumably searching for answers to some of the same questions – and many of us are still looking. The bottom has now dropped out of the colouring book market, with Forbes declaring it “dead” in May, and, in June last year, Headspace laid off 13 staff members.
This involves relaxing your muscles in a two-step process. Identify each muscle group, starting with your toes and moving up.  First tense the muscle or muscle group. Then release and relax them. Notice how you feel as you release the tension. You can do this with your fists, neck, shoulder and pretty much any other muscle you are able to control. But progressively moving up the body, one muscle group at a time, will be most effective.

Relaxing is increasingly difficult in our always-on digital world. This first struck me a couple of years ago when I had to stop exercising after an injury. Exercise had always been my go-to “me-time” activity, and without it I felt totally lost. I recently started again, but having only one means to de-stress now feels very limited and I am not even sure it counts as relaxing – it is quite hard work, and inherently competitive. When I find myself at home with a free evening, I often have no idea what to do and inevitably end up staring emptily at one screen or another for hours, before stumbling off to bed, wondering where the time has gone.
It is important to recognise the difference between good stress and bad stress, life-changing events like getting married, becoming a parent, moving house, starting a new job or starting school can all be stressful events; but these are ultimately good for you – the stress is usually short-lived.  Bad and more dangerous stress is long-term background stress which can be caused by any number of factors including: frustration, overwork, failure or perceived failure as well as more specific life circumstances and illnesses.

In the summer of 2009, I attended the Science-Based Medicine conference and The Amazing Meeting 7 in Las Vegas: a huge gathering of skeptics, scientists, and critical thinkers. I was the only alternative health care professional at the conference that I know of. Like massages, chiropractic care can be beneficial for lowering pain and improving recovery. It’s also been shown to lower stress, headaches and more. One way that chiropractors help treat pain is by lowering mechanical compression and irritation of spinal joints, which can send nerve signals throughout the body that increase inflammation and irritation.
Going to sleep on time and spending seven to nine hours in bed does not ensure that you get enough sleep. The reason the Pressure Question exists is that it’s hard for patients to tell the difference between nasty pain that might be a necessary part of therapy, and ugly pain that is just abusive. Not everything that hurts is therapeutic, but not every therapeutic procedure is painless! How can we tell if an intense massage technique is therapeutic or not?

A licensed, traditional massage practitioner is required to complete at least 800 hours training.[2] Massage therapists must acquire a professional license and must register at the Public Health Ministry's Department of Health Service Support (HSS). In ashiatsu, the practitioner uses their feet to deliver treatment. This deeper level of relaxation improves the individuals' personal outlook, and their emotional status. Deep relaxation has been shown to promote a deeper, more restorative night's sleep as well. This deeper sleep allows the body to heal itself physical, and let you feel more refreshed upon awakening.

Does massage therapy “work”? What do massage therapists say that they can do for people and their pain, and is there any scientific evidence to support those claims? Massage is a popular treatment for low back pain, neck pain, and tension headaches — can it actually treat them, or does it just pleasantly distract patients and maybe take the edge off? In this article, I examine massage therapy in the light of science — not “objectively,” but fairly.1 I go out of my way to be critical of my former profession — I consider it an ethical duty. Health professionals must be self-critical and critical of each other: that is how we improve.2 And, alas, massage therapists are guilty of an astonishing amount of bullshit.


Not everyone should receive a deep tissue massage. Some people simply enjoy the sensation of deeper pressure to their muscles and others prefer a more gentle touch. Someone who has never experienced a massage before may not want to request a deep tissue massage. It is the responsibility of the massage therapist to determine if a deep tissue massage is necessary by way of a thorough health history and evaluation. A massage is only effective when the person on the table is comfortable and relaxed.
