Applying pressure to tender muscle fiber adhesions (known as "muscle knots") can hurt, but there is a difference between that type of discomfort and pain due to excessive or inappropriate pressure and stretching. While it may be difficult to avoid some discomfort when targeting muscle knots, a qualified, licensed massage therapist should be able to adjust the massage pressure and movements so that you are not in pain.
On the other hand, hands-on experience is valuable too, and anyone who’s had a good massage is certainly familiar with the seemingly magical way that a therapist can find the perfect spots and “good pain.” An inability to reliably find something does not mean it can’t be done. And the main reason no treatment has ever been proven effective is mostly a lack of good science on the subject — it’s just unknown.
People think of massage therapy as a “safe” therapy, and of course it mostly is. But things can go wrong. Serious side effects in massage therapy are rare, however, and common side effects are minor. A 2007 survey of 100 massage patients49 found that 10% of 100 patients receiving massage therapy reported “some minor discomfort” in the day following treatment. This would mainly be a familiar slight soreness that is common after a massage — I’m surprised only 10% reported it. The massage must have been quite gentle.
Thai Massage is a popular massage therapy that is used for management of conditions such as musculoskeletal pain and fatigue. Thai Massage involves a number of stretching movements that improve body flexibility, joint movement and also improve blood circulation throughout the body. In one study scientists found that Thai Massage showed comparable efficacy as the painkiller ibuprofen in reduction of joint pain caused by osteoarthritis (OA) of the knee.
I am also a triathlon coach and personal trainer, so I mix and match my appointments every week in between massage sessions and coaching and training sessions with my clients. I typically have between five and eight massage sessions per day, four to five days per week. There are typically two sports massage sessions per day. Most massage sessions include corrective exercise review so the client knows what self-care they should perform.
Although a lot of Bastian 2014 is certainly relevant to the concept of “good pain,” strictly speaking I don’t think they are writing about the good pain paradox, which is defined by simultaneous pleasure and pain. They are writing about pleasure following pain (relief from pain). This is more comfortable scientific ground: it’s pretty straightforward that relief from pain might be “associated with positive consequences” or lead to “activation of the brain’s reward circuitry,” for instance. One theory is that muscle knots may be caused by something that goes wrong at the “motor end plate” — where a nerve ending attaches to a muscle cell.9 We don’t know why this happens, or what exactly goes wrong, but there is circumstantial evidence that motor end plates are the “point” in trigger point. That evidence is too complex and controversial to review properly here. It is explored in detail in my book. Some research has suggested that it may actually be possible to physically destroy the motor end plate with strong massage, thereby inactivating the trigger point.10 When it regrows — these are microscopic structures, it doesn’t take them long to heal — the trigger point may be gone.

When your brain is sleep deprived, it may not be able to respond to emotional events appropriately.
